If  your child is looking ahead to participate in athletics this year, remember to take care of necessary paper work early. In order to be eligible to participate, an athlete must have the following forms on file: QJH Activities guidelines, assumption of risk form, proof of insurance, treatment authorization form, a current physical, and a current ASB card. It  saves everyone a lot of time and headaches if your son/daughter can get these forms turned in before school starts each Fall. The forms are always available in the QJHS office.
